Medical Records are not required at your first visit. Please bring your drivers licence, Thank you.

Telemedicine Directions for Patients
ReCertification Patients (New and Previous):
Step 1:Book your Telemedicine Appt. Online
Step 2: All Recerts (Previous pts. and New pt. recerts) before your appointment, review the drwalkerwellness paperwork (to discuss later with provider if needed)
Step 3: Read the HIPPA Notice and the Release and Acknowledgment Paperwork.
Step 4: The Provider will call you at the start of your Telemedicine Appt. time. FYI (Provider's phone number may be Unknown or Blocked for privacy). The Provider will verify your identification and ask you to agree and understand your Telemed Appt, the HIPPA Notice, and the Release and Acknowledgment Paperwork.
Step 5: The Provider and Patient will continue Telemed Appt.
New Certification Patients:
Step 1:Book your Telemedicine Appt. Online
Step 2: Before your Appt. Print/Download, Fill Out, Sign and Date the Dr Walker Wellness New Patient Paperwork, HIPPA Notice, and the Release and Acknowledgment Paperwork.
Step 3: Please Scan or take a Photo of all of the forms and Email them to drwalkerwellness@gmail.com for review during your Telemed Appt.
Step 4: To Verify Patient Identification, please Scan or take a Photo of your ID or Licence and email to drwalkerwellness@gmail.com
Step 5: The Provider will call you at the start of your Telemedicine Appt. time. FYI (Provider's phone number may be Unknown or Blocked for privacy).
